Mysti’s Adult and Family Services Featured on California DHCS PATHways to Success Website

We’re excited to share that the California Department of Health Care Services (DHCS) has officially recognized Mysti's Adult and Family Services on its PATHways to Success platform.

The platform spotlights our work in expanding access to sustainable housing services through the Providing Access and Transforming Health (PATH) Collaborative Planning and Implementation (CPI) initiative.

Our feature highlights how Enhanced Care Management (ECM) and Community Supports make real change possible for individuals and families across Monterey, Contra Costa, Los Angeles, and Santa Cruz counties.

Through these programs, our team has:

• Helped clients secure safe, stable housing

• Connected individuals to critical healthcare services

• Provided resources that foster long-term stability and self-sufficiency

• Built partnerships that strengthen community-based care across California
